Work Order Requests

  1. This request must be received by Facilities at least seven (7) days prior to event.
  2. This form is not to be used for emergency requests. Emergencies are to be telephoned to extension 6690 (the person responding will complete a written request.) An "emergency" is a condition threatening peoples' health or safety or a condition likely to cause immediate damage to property.
  3. This form is to be used for unscheduled maintenance, custodial and grounds services. You may also use this form to submit requests for minor capital improvements that are not in the annual budget. A "capital improvement" is a modification or enhancement of the original function of buildings or equipment. "Minor" is defined as requiring less than $300 if contracted from an outside vendor. These projects have a low priority and are completed on a time-available basis in the order received.
  4. Both unscheduled maintenance and capital outlay exceeding the limits of a "minor" project (see preceding paragraph) will be referred to the Vice President, Business Services for approval. Normally, only maintenance projects will be considered for the current year. Projects not approved for completion in the current year will be returned to the originator for submittal as a budget request for the coming year.
  5. A confirmation copy of this request will be returned to the originator within 30 days from the time it is received by the Facilities Department. Minor maintenance requests will indicate an estimated time for completion. Minor capital requests will indicate that they are scheduled on a time-available basis for the current year. The decision of the Vice President, Business Services will be shown for maintenance and capital projects exceeding the limits of a minor project (see above). Denials for completion in the current year may be appealed to the Budget committee.
  6. If equipment or furniture is to be moved, be sure that the item is left unlocked or arrange to deliver the key to the Facilities Department (Oceanside Campus, Building 4200 or San Elijo Campus, Facilities). Facilities staff are instructed not to move items that cannot be unloaded or disassembled to move safely.
  7. Include the time that you would like to have the service done. Be as flexible as possible. Allow maximum time for scheduling of the service. It will increase the probability that it will be done when you want it.
  8. State the exact location of the service to be performed, including building and room numbers and/or diagrams. Labeling items to be moved is helpful.
  9. SCHEDULING: Work Order Requests are prioritized in the order that they are received and processed. At certain times of the year, workloads are heavier than at other times. During holidays, spring and summer breaks, work schedules are typically more intense as we attempt to access areas that are difficult to reach when classes are in session. Because we aim to be responsive to the needs of the college community, we ask that you submit Work Order Requests well in advance of the target date that you would like the work completed.
