Important Forms for IIP Students
- Special Request Form: You need to download this form when you request travel signature on your I-20, letter of verification of enrollment, request for Social Security Number (SSN) and request for concurrent enrollment or reduce full-time courseload. Please return this form to the IIP when you file a special request.
- On-campus Employment Agreement Form: You must fill out this form in order to complete your on-campus employment hiring process. Read the form carefully and return it to IIP after completion.
- Curricular Practical Training (CPT) Form: This form has information regarding the eligibility and application process for CPT. You need to fill out information regarding your CPT employer. Both you and your academic advisor need to sign on this form before returning it to the IIP.
- Optional Practical Training (OPT) Application Forms: This link provides step-by-step OPT application procedures. You can also print out the USCIS forms including I-765 and I-538 for your OPT application.
- Official Transcript Request Form: Download this form if you need an official transcript from MiraCosta College. Fill out the form and follow the the instructions listed on page 1 before returning it to the Admissions and Records (A & R) Office. You can mail or fax the form to the A & R Office located in building 3300. You can also request a transcript online to save time!
- International Student Tax Form 8843: All international students are required to file Internal Revenue Service (IRS) Form 8843 even if you do not earn any income in United States. If you earn income in United States, then you also need to file Form 1040NR-EZ in additional to Form 8843. The forms are available on the IRS web site.
